Patient Health Questionnaire -- Depression Screener

These are the nine questions that make up the PHQ-9 -- one of the most widely used depression screeners in clinical practice. Answer for the past two weeks. Your gut is usually right.

Over the last two weeks, how often have you been bothered by any of the following problems?

Question 1 of 9Little interest or pleasure in doing things
Question 2 of 9Feeling down, depressed, or hopeless
Question 3 of 9Trouble falling or staying asleep, or sleeping too much
Question 4 of 9Feeling tired or having little energy

Question 7 of 9Trouble concentrating on things, such as reading the newspaper or watching television
Question 8 of 9Moving or speaking so slowly that other people could have noticed -- or the opposite, being so fidgety or restless that you have moved around a lot more than usual
Question 9 of 9Thoughts that you would be better off dead, or of hurting yourself in some way
